Northern Sierra Rural Health Network
The Northern Sierra Rural Health Network (NSRHN), an entity now part of the Health Alliance of Northern California, needing a partner to assist them with implementation of a Health Information Technology (HIT) software solution for monitoring their patients with diabetes and improving care to this chronic condition population, published a request for proposals.
Lumetra Healthcare Solutions was the perfect partner, bringing decades of experience with California rural health providers, improving their capacity for HIT adoption, their culture for patient safety, and their quality improvement processes and outcomes. Winning this proposal, we worked alongside the management team at NSRHN for one year as they visited their several clinics throughout California, designing an implementation schedule for the new software program, strategizing integration with existing HIT in the clinics, and incorporating clinical quality improvement activities to help analyze improvements in processes and outcomes.
With a variety in operations and baseline readiness at each site, the project year concluded with several clinics utilizing the new system, incorporating quality improvement monitoring to demonstrate improvement in care for patients with diabetes, and coaching their peer clinics to achieve the same outcomes.
