Lumetra has scheduled a Web conference on Tuesday, April 21 at 12:00 noon Pacific entitled: The Patient Centered Medical Home: What Does it Mean for Clinical Practice? The one-hour event is provided at no charge.
This Web conference will introduce participants to the patient-centered medical home, which is an innovative model for primary care that is structured to provide superior quality of care in physician practices by delivering comprehensive, ongoing, and coordinated care for people of all ages and all clinical conditions.
Joseph E. Scherger, MD, MPH, who is the Consulting Medical Director – Quality & Informatics at Lumetra will conduct the call. Dr. Scherger is a Clinical Professor of Family & Preventive Medicine at the University of California – San Diego. His primary focus is the redesign of the medical office practice using the tools of Health Information Technology and the methods of Quality Improvement. He is a member of the Institute of Medicine and served on the IOM Quality Committee, which wrote these reports: “To Err is Human” (2000) and “Crossing the Quality Chasm” (2001).
After attending this Web conference, participants will be able to accomplish the following:
- Define the Patient Centered Medical Home (PCMH) and the criteria being used by the National Committee for Quality Assurance (NCQA) to measure the PCMH.
- Implement the core principles of proactive care by a team separate from office visits.
- Describe the key elements of the redesign of clinical practice to be a successful PCMH.
- Discuss how the medical home model will better serve various cultural and ethnic groups.
